Before filling out the form, please take a moment to read the booking and deposit information below:

A non-refundable deposit is required to secure your appointment. This deposit covers the artwork for your appointment and compensates for the time spent creating a custom design. While the deposit is non-refundable, it will be applied towards the total cost of your tattoo session. If you need to reschedule, please provide at least 3 days notice; otherwise, the deposit will be forfeited.

I create the tattoo design in a single version, and we will discuss all the details beforehand to ensure we are on the same page. Therefore, please take the form seriously when filling it out. Significant changes to the design cannot be made once the artwork is finished, and the seriousness of any changes is at my discretion.
